Multiple Ways to Access Care Management Support

You can be referred to Care Management in two ways: through your insurance provider or your KPPC provider. Once referred, our team will connect with your family to begin the support process and next steps.

Personalized Support Tailored to Your Family

A care coordinator will then reach out to discuss your family’s needs and next steps. With your consent, a care manager will complete a phone assessment that includes health, social, and lifestyle questions to better understand how we can provide personalized support and resources for your child and family.

Individualized Care Plans with Close Follow-Up Support

A care manager will create a personalized care plan with specific goals tailored to each patient’s needs. The care team then provides close follow-up and ongoing support over a 2–3 week period to help ensure progress toward those goals.
