    Transfer Medical Records

    If your child has been seen by another pediatrician, we’ll help coordinate the transfer of medical records. This ensures your provider has a complete view of your child’s health history, including immunizations, past visits, and any ongoing care needs.

    Complete Patient Forms & Meet Your Care Team at Your First Visit

    After forms are completed and accepted by our team, and you’ve initiated a records transfer, you may schedule your child’s first appointment. Appointments can be scheduled over the phone at one of our many locations. Once scheduled, patients will receive a confirmation phone call 24-hours prior to their visit.
