  • Vaccine Policy

    Our practice is committed to following the recommended immunization guidelines to help protect the health and well-being of all children. Vaccines are an important part of preventive healthcare and are recommended according to established medical standards.

  • Tardiness & No Show Policy

    Please arrive on time for your child’s appointment, and new patients should arrive at least 15 minutes early to complete registration.

    If you need to cancel or reschedule, please provide at least 24 hours’ notice. Patients who miss three appointments without notifying the office at least 24 hours in advance may be dismissed from the practice so appointment times remain available for other families.

  • Release & Search of Medical Records Policy

    Routine requests for prescriptions, physician callbacks, forms, and other documents are typically completed within 48 hours. Please allow adequate processing time when requesting medical records or other documentation.

  • Custodial Situations

    Parents or legal guardians are responsible for providing any legal custody documentation that may affect a child’s medical care or access to medical information. Without court documentation on file, both parents may have equal access to their child’s medical information and participation in healthcare decisions, as permitted by law.

  • Age of Consent

    Patients who have reached the legal age of consent are responsible for making their own healthcare decisions and authorizing the release of their medical information, unless otherwise permitted or required by law.
